Five simple steps

- Download and complete the Affidavit of Service for the notice
- Sign the Affidavit of Service in front of a Notary Public.
- Complete the online form. The fee is $379 if not completed online
- Mail, fax or drop off a copy of the 5, 14 or 28 day notice and the completed affidavit along with payment at our office.

Please let us know in advance of court if you are willing
to let the tenant enter into a court ordered payment
agreement. It is also important that we are advised of any
payments you have received from the tenant after the notice was given to the tenant.
